This Frankenstein Monster Mask ad could be found on page 46 of the first issue of Fantastic Monsters Magazine.

The mask appears to be a Don Post Studios B-1 Frankenstein Monster Mask. It's a bit hard to tell as the face is slightly misshapen, but the forehead scars seems to be consistent with the catalog photo of the B-1 pictured below...
